I have moved all of my accounts to my own mail server: Apple Mail Server
running on Mac OS X Server 10.2.6.
I have been leaving messages on the server and have about 4000 messages
there. Just last week I added to my account prefs that messages should be
removed after 30 days and also when deleted locally.
After this I keep getting smaller amounts of old messages that I have
already received. In fact the server have marked them as such by adding
"Status: R" as the first header line.

I haven't yet investigated the connection stream as I wanted to give
Jérôme a heads-up that we now have the possibility to look at earlier
reported similar problems from both sides. I have had problems at several
other mail servers, ie if I keep messages and don't delete them, already
fetched messages keep coming back.
This is true with the account I get my PowerMail discussions list, which
I'm forwarding to my server. Before the changes described above, I could
leave messages and PowerMail had no problems with that.
