I've mentioned this before, but I had some long discussions with the developers of Apple's Mail application when I was working at Apple. They mentioned to me that they don't have true threading, and this is because not every mail application puts threading information in the header (which makes since since there isn't any RFC standard for it in email). They use a combination of subject, date, and server data to try and sort out the message threading. They don't consider it 100% reliable. I'll have to go back and see if I can find my old messages on this subject because they included some additional details.
