We went with an outside filtering service (Postini). The filtering is not perfect, but its quite good and easy to use. Since we are a small company, we got it through a VAR. Price is $2 per human user per month, with a minimum of 10 users, plus a few hundred $ initial setup (but with a free trial). It also does a great job on viruses, which was great during some of the recent Windoze outbreaks, where it kept hundreds of megabytes per day from reaching our server.
