Any information you can provide is greatly appreciated. I have many questions
about industry best practices with regards to Hot Spots and/or elevated dose
rates.

1. Do you use a special system to prioritize your Hot Spots for removal,
Shielding, Flushing, etc?

2. If you have a priority scale, how many categories of prioritization do
you use. ( Example: High priority, Medium and Low priority.)

3. Does your plant recognize a threshold for hot spots? I.e. If a source
is more than 100 mrem/hr at 30 cm is it considered a High Radiation Area only
or do you still consider that a Hot Spot also?

4. If you find a source that meets your plants criteria for a Hot Spot
by dose rate, but it is in an area that is considered "In-accessible", do you
consider that a hot spot or an elevated dose rate? And, how do you track that
hazard, as a Hot Spot or an elevated dose rate source?

5. How many Hot Spots does your plant currently have?

6. Do your procedures require that you tag every hazard identified that
meets the hot spot criteria, regardless of where it is located? I.e.
inaccessible areas. Or do you only tag/ post accessible Hot Spots?

Thank you,

Dave
