1.                    When a worker receives a medical exposure to a 
radiopharmaceutical, how do you survey of the individual?  Passive monitoring?  
WBC?  Frisker?

     *   No survey if individual self-reports prior to entering RCA.  Upon 
reporting, individual is restricted from entering RCA and advised not to wear 
dosimeter - estimated duration is given if the nuclide is known.  If dosimeter 
had been worn, it is collected and a new one is issued once they can clear 
PCMs.  Individuals are permitted to bypass PCMs when exiting the gatehouse 
until the isotopes decay.

2.                    How do you document the survey?  VSDS?

     *   If an individual hadn't reported in advance and were to get stuck 
exiting the RCA, a whole body count would be performed to determine whether 
non-medical isotopes were present.  The WBC includes their name and ID# and is 
permanently placed into their dosimetry file.
3.            Do you identify the individual in the documentation?  By name? by 
a unique identifier (other than their social security number)

By procedure workers are expected to notify RP that they have had medically 
administered radionuclides prior to RCA entry. They usually come in to the 
dosimetry office prior to entering through the access facility. They usually 
bring documentation or we have them call the doctor's office. There is an 
expectation that they do not wear their DLR after their test. If they have worn 
it we take it from them.

They are not allowed to go into the RCA or wear a DLR until the radionuclides 
have decayed. We sometimes use our passive monitor in the dosimetry office to 
check them depending on the isotope. We issue them a pass to bypass the exit 
contamination monitors.

  *   We do not normally survey the worker unless they have entered our 
Protected Area without prior notification and have alarmed our contamination 
monitors.  Workers that have had a radiopharmaceutical treatment will report to 
HP and provide documentation of the nuclide and the activity.  We have a 
process for workers who do not enter the RCA that allows them to bypass the 
exit contamination monitors.  Workers requiring RCA access will require 
contamination monitoring (PCM and frisker) and a analytical WBC to allow 
releasing them from the RCA.

2.          How do you document the survey?  VSDS?   No survey in the 
traditional sense.
3.          Do you identify the individual in the documentation?  By name? by a 
unique identifier (other than their social security
             number)? - We use a unique identifier and their last four digits 
of their SSN.
