This is a minor update. Aside from small editorial fixes, the only
changes are:

1. Added an explicit open issue about spaces in usernames and passwords
(for saslprepbis).

2. Added two additional IANA registries: one for base classes (with a
policy of "RFC Required") and another for subclasses of the base classes
(with a policy of "First Come, First Served"). I debated whether to
specify a less formal policy for the base class registry (e.g.,
"Specification Required"), but given the seriousness of adding a new
base class I think "RFC Required" is more appropriate. Let's discuss.
