With top-posting you still need to scroll down to get the context.

There is a plethora of mail handling applications out there - old, new, ancient,
graphical, text-based - personally I would recommend aiming for the lowest 
common
denominator, not just the "right" ones.

Yes, no difference in effort. However, to me, it just doesn't make sense to 
do anything other than interleaving quoted text with replies, the replies 
immediately following the relevant quotation.

Presumably anyone who considers they know best! Usually those who call 
themselves type-1 have someone else read their mail for them anyway, they 
consider themselves too important to do menial tasks themselves.

Precisely.

IMO the etiquette problem that some people have is not cropping bits of the 
quoted message which are not referenced in the reply.
