I've been compiling a list of "things that patient has the right to request 
and other privacy stuff the provider is obligated to keep track of".  I 
would assume that the provider will want his office management application 
to have fields for this... maybe a HIPAA-preferences table.  Any automatic 
messaging system that selects patients for a particular disclosure will 
have to respect these preferences to keep the doc. out of trouble.
Here's what I have so far:
-Patient willingness to accept each type of unsolicited info the provider 
might want to send out(e.g. marketing info, order status messages, TPO 
information, appt. reminders, advice of a need for a service, etc.)
-The type(s) of communication (mail, phone, email, etc.) patients will 
accept or will not accept for each of these message types.
-Special preferences about disclosures to other family members, news media, 
etc.
-Consent type (verbal, written, etc.)
-Special authorizations on file
-Non-routine disclosure log.

Having this automated in the provider system would facilitate an interview 
or wizard-like interface for the patient to complete a "privacy 
questionnaire"... thus fulfilling the need for the "privacy conversation" 
with less effort.  This privacy preferences profile (PPP?) could be 
maintained by patients via a web site and a copy given to the patient for 
approval/general information when he/she presented for an office visit.

Similar DB support should be in the OMS for Busines Assoc. agreements for 
each BA... date entered, type of contract, paper/electronic, etc.
