Melissa - a tool was developed at my last health plan client site. It isn't proper for me to share the tool, but I can highlight what we were trying to capture:
Focus was on PHI flow - uses and disclosures The uses are to be identified and measured against the minimum necessary standard in order to help us determine the common data categories and employee classes needed to estabish our role-based access environment. Disclosures are analyzed in much the same way for minimum necessary but we also wanted to know if they were routine and recurring or not. Those that were nonroutine or nonrecurring are to be reviewed with a bias towards dropping them altogether. The other aspect we wanted to capture was whether or not the disclosures fell within or outside our definition of TPO. Again, we are strongly discouraging the non-TPO disclosures. Our stance is that the exceptions will have to be reviewed and approved by the PO.
