We recommend that CEs buy a comprehensive set of P&P templates.  Use the
table of contents as a guide to assemble your current P&Ps that have any
bearing on the subject matter.  Then compare your policies with the
templates, and make changes.  This will also help highlight missing
policies.

Alternatively, you can make your own list of HIPAA requirements, to use in
lieu of the template table of contents.  The trade off is the cost of the
templates versus the cost of your time.

Or e-mail me off list and I'll send you the table of contents from our
templates, no charge.
