Below the histogram, beside "Settings" are two flyouts. The right hand one (round button) contains "Reset Camera Default". But as you've probably tried that, my only other suggestion, which applies to all technology, is turn it off, then turn it on again. If still faulty, uninstal Photoshop, manually deleting all associated folders, then reinstall. Don't forget to save your actions and plug ins to another folder.
