Are there any rules of thumb for the percentage of USM (Photoshop CS) to
apply to digital images and is it best to add this after or prior to CMYK
conversion?

Dear Chris

The best solution is to make your conversion to CMYK then sharpen, and then ensure that your end points are set as you would wish bearing in mind all the considerations that pertain to your chosen printing method/conditions.

The amount of USM you use and how you apply it is job/subject/print condition and personal experience choice dependent. Sorry to be so vague but every job is different and there is no standard fit all answer...just that you should be aware of the considerations. It sometimes helps to view the file at 50% when working with USM.

Hope this helps.
