Malcolm: I use West Wind HTML Help Builder. It gives me .CHM files that I distribute with my applications and also generates a nice Word document that I use as a manual at the click of a button. I can send you a sample of what it looks like if you ask me offline. You can also change the Word template to fit your individual preferences, but I like the default just fine.
