If you have an Exchange or SQL agent for your AV program, you definitely don't want real-time scanning of the physical files that contain the DBs. In the case of Exchange, that would be *.stm and *.edb. At least for 2003. I think the file extensions are the same for 2000 but I don't know about 5.5 as I never had the misfortune to work with it.
