"Deliberate" is a matter of interpretation, no? I did lose a gig some years ago when the recruiter who was paying me announced that I had to have my own Errors and Omissions policy. It was still 1099 work and they even specified the coverage they required. The only policy I could find that came up to their specs cost $18k/yr.
