SR will prolly chime in here soon, but in the mean time.... A while
back he mentioned a command line tool (name escapes me at the moment -
pdfsam? ) that does what you want.

When I tested it I just put some command in a DOS BAT file and it
worked well when I was combining, slicing and dicing dozens of PDF's
at a time.
