That's largely due to Microsoft's influence, thank you very much. Their
requirements (upper/lower case, number, special characters, frequent changes)
were a step in the right direction back in the 90s, but are obsolete now. The
problem is that too many people coming into computing learned those and never
advanced their knowledge as the field progressed (see http://xkcd.com/936/ for
an excellent illustration).
