The only place I find the new pdf form is at the NUCC site but this pdf form seems a bit smaller than a complete page specially at the bottom of the page. The form just came final a few weeks ago and I need to work on these and have it ready before the March 2007 deadline. Does anyone have this new form either in bmp or gif or can lend me a hand to convert and resize this new form? I have a pdf converter but the results are far from at least acceptable. Regards.
