I have had this when the 2 foxuser files are mismatched (i.e. one of them gets 
updated but not the other). Deleting them both and letting the Launcher 
replacing them ususally fixes the problem.

I thought your scenario could be similar.

Thanks to all who responded.  There was definitely not another copy hanging
around as we deleted them all.  It is not permissions either.  The good news
is - the problem as gone away!  The client replaced the foxuser files with
copies from a backup and all is well now.  Most peculiar!
