I was at a company in Philly 3 years ago where that was the case...it 
became terribly corrupt and rather than deal with it, they just kept 
putting it off.  What a nightmare.  The entire mgmt in that team 
sucked.  I left after 4 months...couldn't stand the inept leadership.  
Sadly, I here the same boss is still there, and there's still tons of 
turnover in her group.  Don't you think upper mgmt would get a clue 
about what the *real* problem is?  Ugh.
