I use Rick Strahl's West Wind HTML Builder. Not only to help build end-user
doco, but to create technical documentation for databases, VFP classes, and
.NET stuff if you are doing that these days. You can even automate the tool
using a COM interface if you have the need to automate the help creation.
Also outputs to Word for the "manual" side of things.

Help and Manual is nice too.
