I use Lulu to print my book (and I saw one of the copies my students
bought - and it looked good). 

I've used Cafepress to print a Calendar(for family purposes) - image on
shirts for my e-commerce store (printing looked good - but, shirt
quality was HORRIBLE) and printing of mousepads (and I ordered several
and they DO Look good). 

But, never did CD's with either of them though...

We're using Lulu for our printed manuals. Took us several
iterations to get things right, but we're pleased with the
results.
