IMO I am not in favor of fourth normal for name or phone numbers. I have that in one of my demographic systems for our company. Yes lastname is a key to the Names table. Firstname is as well, to the same table. For me Stephen Russell that Russell would be only listed once. This is a RPITA and I have a project to mimic data from one environment into our system, talk about torture! I have it this fall.
