SDT is essential for anyone who needs to support DBFs. I'm also a big fan of xCase, but for different functionality. We use a couple of their diagrams nearly every day, though in MySQL now. It's a little-known feature that xCase will also print reports, too. And it has the ability to pull in database structures, sometimes relations and comments, which can be a real time-saver. xCase also supports all the different database engines you mention, and a grunch more.
