What model of printer is it?   

Sounds like the print driver to me.  Try a different model of printer, a 
compatible printer, or an updated driver.  The print driver on the server or 
the client could be suspect.  The clear indicator is that the pdf writer works 
fine, which indicates its a printer driver issue.  You might also try the "raw" 
setting.   

I've had similar issues with inkjet printers.  At one installation instead of 
using the driver that came with the printer, I went to an older, simpler driver.
