The bibindex side has been implemented. idxPHRASE are now filled at indexing
time. Still missing the search engine to exploit them.

This task is connected to activating idxPHRASE* phrase indexes
during bibindex process. (Currently we are treating only word indexes in
bibindex and we leave phrase indexes onto MySQL bibxxx tables.)
