Well I've fixed the problem and it is completely unrelated to mozdev
downloads and the updateURL.
For some reason Firefox keeps a temporary copy of some extensions in a
folder called staged-xpis within your extensions folder. Within this
folder was another folder named with my extensions GUID containing 4
temporary versions of my extension, 2 of v0.2 and 2 of v0.3. I think
that when firefox tried to install my extension it kept picking on of
these temporary old copies instead of the new version.
I can't find any documentation from mozilla indicating how this problem
might arise so hopefully it only happens once in a blue moon.

i'll be interested to see if I have the same issues when the latest
version gets added to mozilla addons or if its related to the mozdev
version somehow. only difference is that the mozdev install.rdf
includes an updateURL. could this affect things in any way?

Try installing to a new profile just for kicks. Also, open the XPI and
check the version number specified in install.rdf. This will tell you
which version you've *really* downloaded.

you give below and 0.2 still reappears on my system.
I checked my extensions folder after uninstalling it and there was no
trace of it there, is there anywhere else where my system could be
finding the 0.2 files from?
