In my opinion.

There needs to be more than two levels on AMO. At the moment there is no middle ground, either you are in or out, public/sandbox. Say you have 5 levels:
1 is sandbox
2 after automatic checks for error, bad code etc
3 author has provided reliable means of identifying them as a real person
4 good feedback and has been checked by reviewer
5 has been around with no problems for 6 months
Every update to an existing extension will go to 1 until the automatic checks are done then it will jump to whatever level the previous version was at, except level 4, in which case it would go back to 3.
Each level would have an appropriate notice on the download page.
This would need to be refined.

Having an automatic process of release for new extensions should never be considered, all someone with bad intentions needs to do is flood the sandbox with extensions for review and overload the reviewers. Bad extensions will then be pushed public...

I have my TimeShift extension on AMO, I will not push it for feedback or review for a few reasons, first I don't care if anyone uses it because I did it for me, second there are other more useful extensions that I would love to see moved out of the sandbox.
