The fiducials are optical 'datums'.  When the part placement equipment
detects the fiducials, it is calibrated to locate the rest of the parts.
This seems especially essential to machines which move the board to place
parts in different locations.

Well, that is one brief explanation, and there are a number of different
machines and requirements ( i.e. some shops want holes for alignment pins ).

I have been placing round 40mil pads (sans hole) in 3 corners of the layout.
I enlarge the solder mask clearance and null the paste mask (check the
override box and enter -21mil in 99SE).  No complaints yet,  I have only
used fiducials on SMD.  No requests for it on through hole boards.
