Fix For: 0.8
Windows IOCP functionality relies on the very handy refcounting capabilities of
the maps and lists. The usage conventions have change considerably and broken
the Windows IO at many levels. Update for these semantic changes and
r1625220's switch from pn_new() to calloc().
