You're going to kick yourself. ;-)  It's because you're submitting the
form, so the page is getting refreshed after the elements are added.
Change the submit button to just a button and the behavior will go

OT: You probably want a tbody in that table, otherwise things will be
a bit unreliable.  TR elements must be inside TBODY, THEAD, or TFOOT
elements in the doc type you're declaring, they cannot be direct
children of TABLE.  Most browsers will let you get away with it in
your original markup (silently adding the TBODY for you), but if you
do DOM manipulation, you'll run into trouble.
