This change is required to efficiently calculate cronbach's alpha.
A new function casereader_create_append_numeric which creates a new
casereader, one variable wider than the input reader. The caller provides a
function which must return the value of the new datum.
This change generalises Ben's arithmetic-sequence case translator, and
re-implements the latter in terms of the former.
