In the VoiceOver Utility, set your mouse pointer to follow the VoiceOver cursor. Sometimes Control-Option-Space Bar will work fine and occasionally you'll need to use the added Shift key. Not a big deal. You'll be better off with that setup when you need to do a physical mouse click for modified clicking on certain controls.
