You need to override chunk-element-content for html and process.root for 
html-single, then you can wrap the calls to header and footer with your div.

Also you should beware of your xsl:imports, e.g. your html.xsl files has:

<xsl:import 
href="http://docbook.sourceforge.net/release/xsl/current/fo/docbook.xsl"/>

This is the file for FOP output and probably won't work with the publican 
common XSL which uses:

<xsl:import 
href="http://docbook.sourceforge.net/release/xsl/current/xhtml/docbook.xsl"/>
