The only explanation that I can think of is that the card gets somehow permanently mis-initialized if pulseaudio tries to use it too soon. Some things to test that theory: once you have done a failed hotplug attempt, and left the card plugged in, does it also fail if you unload module-udev-detect and load it again? If that fails, does it fail if you restart pulseaudio (without unplugging the card at any point)?
