Well, the ghost card disappeared. I've tried restarting PA a couple of
times, and after a reboot it still didn't reappear. So if you hunch is
correct then this must have been a HAL quirk (maybe related to
hibernation?).
I'm closing this bug. If someone else stumps on it again he'll hopefully
find and re-open it. I can't really be of any help without being capable
of reproducing it.
