#681: need to support cards which have 1 audio and 1 modem pcm

we need to properly ignore modem pcms, but not the entire cards they are
in:
https://bugs.launchpad.net/ubuntu/+source/pulseaudio/+bug/394500
also, we need to check udev's SOUND_CLASS not SOUND_FORM_FACTOR env var
for modems.
