#908: Mark Hot-plugged Devices as Default

Pulseaudio needs the capability to set hot-plugged devices as default
sink/sources.
As far as I know, set set-default-sink, set-default-source only works if
the devices are present during daemon startup.
Use cases:
*USB Sound Cards* -> laptops
I'm tired of opening up pavucontrol and marking my usb sound card as the
default sink every time the usb cable wiggles the wrong way or I want to
move across the room or when resuming from sleep.
(On a side note, usb sound devices should be marked as default - as
windows does I think - because they are obviously inserted with conscious
effort by the user and with the end goal of producing sound.)
