The augeas type's implementation and support of the augeas command "ins/insert"
is broken. Among other failings, provider/augeas/augeas.rb calls
aug.insert("single parameter") when aug.insert is expecting 3 args.
See attached patch which fixes provider/augeas/augeas.rb (fixes to make it go)
and type/augeas.rb (doc updates for how "ins" actually works).
