Jira (PUP-5702) Add code_id and catalog_uuid to report

Verifying this ticket, I followed the steps that Josh described above:

In '/opt/puppetlabs/puppet/cache/client_data/catalog/c0isjw8u2wlb88c.delivery.puppetlabs.net.json' on my agent I see:

"code_id":"cats","catalog_uuid":"c3ac4391-ef76-425f-881f-9c42939d773d"

And in /opt/puppetlabs/server/data/puppetserver/reports/c0isjw8u2wlb88c.delivery.puppetlabs.net/201601270049.yaml on my master:

catalog_uuid: c3ac4391-ef76-425f-881f-9c42939d773d

code_id: cats

This was after I made the following changed to '/opt/puppetlabs/puppet/lib/ruby/vendor_ruby/puppet/resource/catalog.rb' to set @code_id:

    276   def initialize(name = nil, environment = Puppet::Node::Environment::NONE, code_id = nil)

    277     super()

    278     @name = name

    279     @catalog_uuid = SecureRandom.uuid

    280     @classes = []

    281     @resource_table = {}

    282     @resources = []

    283     @relationship_graph = nil

    285     @host_config = true

    286     @environment_instance = environment

    287     @environment = environment.to_s

    288     @code_id = 'cats'
