While looking at #6711, I discovered that the `refresh` attribute of `exec` 
resources doesn't work as expected.

Nope: Puppet will actually run both the main command and the refresh command if 
the exec gets a refresh event. The description suggests that the refresh 
command will replace the normal one in the event of a refresh, which would make 
significantly more sense. 

I can change the documentation to match reality, but this seems like a case 
where reality is wrong.
