It looks like you've got this installed in at least two different locations, and you also have the directory in your path twice.
I have just pushed a commit that attempts to skip loading of duplicate actions. It also looks like you're running an older copy of the repo, based on the logs you're getting. Can you update your repo (and remove your old copies), and try it again?
