I've done a similar thing in my redhat network template, e.g.:
DEVICE=<%= device %>
BOOTPROTO=static
IPADDR=<%= eval("ipaddress_" + device) %>
NETMASK=<%= eval("netmask_" + device) %>
ONBOOT=yes
TYPE=Ethernet
USERCTL=no
HWADDR=<%= eval(" macaddress_" + device) %>
GATEWAY=<%= gateway %>
