Cache can be configured both on iSCSI level and on VM level. Cache on iSCSI level is configured on a storage basis (storage view in Proxmox) which means this setting is affecting all new VM's while cache on the VM level is affecting only the specific VM. What you can see in napp-it is the cache setting on iSCSI level while the cache setting on VM level is only used as an option when starting the VM in proxmox.
