How long are you waiting between the creation at one client and the ls
on the other?  You could be seeing something related to the name
cache, but entries in the ncache should get flushed in 30 seconds.

I'll definitely be able to help you more with the MPI_File questions.
Are you using MPI_COMM_SELF when you open these processes?  Which
version of MPICH are you using?
