I discussed this a little with Bart off-list but I wanted to reply to the list too in case anyone finds it in the archives. It looks like there is a corrupt entry in the database, which probably means dumping and reloading the database to recover.

I ran db_recover a few times from the storage directory and the fsid directory, but I continue to get these errors. Has anyone run into this? What is the trick
to bring it back?
