Thank you for the instructions.   We have only seen this once and it followed a 
hardware failure of one of four PVFS servers.  So I tend to think the file was 
being written at the time of the hardware failure though I can not be certain.  
The issue was noticed after the hardware failure had occurred and not before.

This is an issue that we see very infrequently. We have been unable to 
duplicate it in a manner that will allow us to find and fix the problem. If you 
find a way to reliably create this problem please let us know.

I assume this is corrupted metadata??  Any attempt to access this file with 
utilities like pvfs2-stat or pvfs2-validate report "no such file or directory". 
  Is there a way to recover this file or remove it?   I am tempted to run 
pvfs2-fsck but I can not find any documentation on it.  This is an active 
filesystem.  Would I need to stop all client access in order to run pvfs2-fsck 
and how long would it take?    This is a 146TB filesystem with 11TB in use.

Any advise on fixing this problem would be appreciated.
