I've run into this before. It's very rare. What is the first numbered page which doesn't show up? I.e when does /documents/n-1 work, but /documents/n doesn't. After you find that, find the n.xml fiile. Here, there are two seperate causes.
1. The n.xml file is not there. In that case, it got lost the last time the server crashed. Reconstruct the file from n.old and restart the server and it should be fine. 2. The n.xml file does exist. In that case, it is probably a VM issue. What system are you running? Peace and Luck!
