Thanks Martin, great collection of info. The work you've put together in 
solving this has saved me endless hours of frustration. 

One tip I could share:

I think VM size should not equal file size but should be byte aligned to 
nearest 16 byte value. Codesign also fixes headers it seems -- provided it 
doesn't fail when running.
