There should be no need to return a HRESULT - but note that the nominated return value must be the *first* value returned, with [out] params following in the order they were declared. As a result, method4 really should only need 2 return values - I'm not sure why it seems to want more. If you can provide a patch to the pippo sample that demonstrates your problem I can dig deeper.
